  Comments:
SOURCE - No raw water sample tap; verify that chemical storage hazards are a sufficient distance from the well. TREATMENT - UV unit has no alarm or shut off. Plan Review process will verify whether UV unit is NSF-approved. MONITORING - Need to supply us with two years of TC and nitrate testing results. M & O - Must go through Plan Review; No Emergency Response Plan; No Management and Operations Manual.
