  Comments:
The deficiencies noted are as follows: 1. CT values must be calculated daily. 2. Locate drains for #1 Larch Mountain and #3 Mershon reservoirs and make sure they have flap valves or screens on them. 3. Cross Connection Program: All backflow prevention devices must be tested annually. You must develop a system for assuring that all devices are tested, or their water service is discontinued. 4. Arsenic was not monitored in 2007. This test is to be done annually from the entry point. Please do the test as soon as possible and send the results to the State. 5. Establish written protocols for each operator, other than the operator in direct responsible charge (DRC - Jim for the distribution and Zack for the plant), which describes operational decisions the operator is allowed to make, and conditions under which the operator must consult with the DRC. I have included a handout with more information.
