Cracked concrete slab around wellhead needs to be repaired. 2. Service area and facility map should be developed. 3. Review criteria for "Outstanding Performing" system. 4. Maintain and review copy of monitoring requirements. 5. Well near pool disconnected. If operated, it must go through plan review. Insure proper seal to electrical wires. 6. Verify devices are used for cross-connection control. 7. Monthly batch disinfection not recommended. Disinfect after repairs, loss of pressure, or positive result. Apply for plan review if residual is desired.
