Management & Operations Deficiencies
1. No Operation and Maintenance Manual.
2. No Emergency Response Plan.

The recommendations and comments noted are as follows :

1. The concrete slab around the well head casing is cracked for Firehouse Well. Recommend sealing the slab to prevent possible contaminants
from entering the well source.

2. There is currently no concrete slab around Irish Well. Recommend placing a concrete slab around the wellhead to prevent possible contaminants
from draining down into the ground and entering the aquifer.

3. The protective housing for Irish Well chlorine and soda ash does not have a lock. Recommend providing a lock to prevent possible vandalism.

4. The current pump house stations with Soda Ash do not currently have OSHA required eye wash stations. Recommend installing eye & hand wash
stations per OSHA rules & guidelines in the event of an emergency situation.

5. The storage tanks are inspected annually. Recommend setting up a cleaming schedule (written down in your O&P manual) based upon annual inspection.
