| Comments: 1. The hatch on the small 0.25 Million Gallon reservoir is not sealed properly. The hatch lid seal needs to be replaced with one that is watertight. Also, the rusted paint on the bottom of the lid needs to be removed and cleaned up. 2. Standard Operating Procedures need to be developed for well #5, as they have been developed for wells #6 and 7, and included in the Operation and Maintenance Manual. 3. Develop a decision-making protocols for operators that are not the Operator in Direct Responsible Charge (DRC) in the Operation and Maintenance Plan. This can also be a separate document. Include the decisions that the non-DRC operators are authorized to make, and when the DRC needs to be consulted. See handout given out during survey for more information. 4. This office will need one more additional round of radiological monitoring for well #7 to establish initial monitoring frequencies for these constituents, or four total quarters of monitoring if there is a detection of one of the constituents.
