  Comments:
Deficiencies notes are as follows: 1. Backflow Prevention Device Testing. Water District needs to contact Blue Mtn Lumber Products to verify if there is a backflow prevention device on the water line. If so, it needs to be tested annually. 2. Radiological Monitoring Not Current. Sample 1 quarter for Gross Alpha, and 2 quarters for Radium 226/228 and Uranium. 3. Lead and Copper Monitoring Not Current. Collect 5 samples during summer of 2009. 4. No approved Written Coliform Sampling Plan. 5. No ERP. 6. No O&M Manual. 7. No Certified Operator. Water system no longer has DRC at required level. 8. No Annual CCR
