  Comments:
The significant deficiencies and rule violations noted are as follows: Distribution: 1. A cross-connection ordinance or enabling authority is required in order to provide you the ability to protect water quality from a potential backflow or cross-connection hazard in the distribution. This can be a simple document that provides you the ability to require a device and annual testing for a potential hazard. - Corrected 2011 2. Cross-connection Annual Summary Reports must be submitted annually to the DWP, whether or not the system has any devices to test on an annual basis. - Corrected 3/1/11 Monitoring: 3. Disinfection By-Products (DBPs) sampling must be done every summer, between June 1 and September 30. This is monitoring for total trihalomethanes and haloacetic acids, and is different from the chlorine residual monitoring required. Please include the results along with the distributing Portland’s Consumer Confidence Report to all users. - Corrected 9/27/10 4. A written coliform sampling plan must be developed. Identify at least three routine locations to rotate through every three months, and four repeat locations for each routine. Operation and Maintenance: 5. No operations and maintenance manual. This is required for all public water systems and should include information on sampling, notification through Consumer Confidence Reports. Corrected 5/13/10 6. An Emergency Response Plan and Vulnerability Assessment must be completed. This should include where to obtain water in the event of an emergency, procedures for responding to a coliform positive, as well as emergency contact information. - Corrected 5/14/10 7. Thank you for sending in certification to the DWP that the 2008 Consumer Confidence Report was sent to all users on time. The Annual CCR 8. Chlorine residual monitoring in the distribution system must be performed twice a week, pursuant to updated Oregon Administrative Rules (OAR) 333-061-0036 (9)(a).
