  Comments:
Deficiencies 1. No Cross Connection Control Specialist (OAR 333-061-0070). Community water systems with 300 or more service connections are required to have a Certified Cross Connection Control Specialist administer the water system’s cross connection control program. 2. Annual Summary Report for the cross connection control program not submitted for calendar year 2008 (OAR 333-061-0070). In Community water systems having 300 or more connections, water suppliers are required to prepare and submit a cross connection control Annual Summary Report to the Department of Human Services - Drinking Water Program, on forms provided by the Department, before the last working day of March each year. 3. New 15,000 gallon steel tank was installed without plan approval (OAR 333-061-0060). Construction and installation plans for new systems or major additions or modifications (treatment, pump stations, tanks, etc.) must be submitted to and approved by the Department of Human Services – Drinking Water Program before construction begins. Plan review submission information can be found on our website at: http://oregon.gov/DHS/ph/dwp/planreview.shtml . Please submit plans for the tank for our review as soon as possible. In addition to the above listed deficiencies, I have the following recommendations: 1. Ensure that all reservoir drains/overflows are cleared of vegetation and soil in order to ensure proper operation of the flap valves. 2. Begin recording chemical dosages (mg/l) daily. 3. Clear debris gathered at the top of the Fox Creek dam. 4. Keep pealing paint and corrosion of reservoirs in check (see photographs below) 5. Complete periodic water loss studies to identify un-accounted for water (leaks, faulty meters, etc.). 6. Keep corrosion and leaks in check in the pump stations (see photographs below). 7. Implement an annual flushing program to keep sediments from building up in the distribution system.
