Significant Deficiencies: Source: Sanitary seal not watertight. If needed, cap the faulty water level measuring device to ensure a sanitary seal. Fix leak in adjacent pipe. Distribution: Cross connection hazards not addressed, no records of annual testing of existing devices. Management & Operations: No O&M manual; No Emergency Response Plan Recommendatuions - Monitoring & reporting: Update Coliform Sampling Plan with GWR requirements & terminology Distribution: Develop a schedule for flushing and valve turning; determine whether additional valves are needed to respond to maintenance or emergency situations.
