The deficiencies noted are as follows:

1. Hatch not locked or adequately secured.

Management & Operations Deficiencies
1. No Operation and Maintenance Manual.
2. No Emergency Response Plan.
3. Annual CCR not submitted.

The recommendations and comments noted are as follows:

1. The access hatch to spring 5 needs to be lockable.
2. Repair the east side overflow pipe that is cracked and locate the end of the pipe and clear the area around where it daylights. Verify that it is
screened properly.
3. A summary of your monitoring requirements can be found on page 12. Please maintain a copy of this page and refer to it for future monitoring scheduling.
4. Recommending that your water system be changed from a Community Water System to a State-Regulated Water System due to your current
population statistics. We should review the population numbers of your community in one year to assure the community does not once again grow
into a "community water system".
