The deficiencies noted are as follows:

Monitoring Deficiencies
1. Coliform sampling plan not representative.

Treatment Deficiencies
1. No means to adequately determine flow rate on contact chamber effluent line.

Management & Operations Deficiencies
1. Major modifications not approved by Plan Review.

The recommendations and comments noted are as follows :

1. Make some notations of what corrective action was taken to regain chlorine residual. This will help protect system liability in the event
that a waterborne disease outbreak occurred.

2. NW Corner Well determined by OHA DWP to be inadequately constructed. Well could serve as a conduit for contamination to move from the
surface or near surface environment into the well. If contaminants such as Nitrates, E-Coli, Inorganic, and Organic compounds are detected,
may be required to install treatment, reconstruct, or find another source.

3. Concrete slab around NW Corner Well is cracked and has 0.25 inch gap around the casing. The east well concrete is cracked and badly
deteriorated. In each case the concrete slab needs to be repaired or replaced to insure that the ground water serving system will not become
possibly contaminated.
