Significant Deficiencies: Vent on reservoir broken. Vents not screened. Emergency response plan incomplete. Recommendations: Check pressure guage in well house, replace if necessary. Well head not protected. Dosage to chlorine feed reservoir not recorded. Well is within 100 year flood plain. Extend well casing to 2 feet above slab.
