Details: I spoke with Bruce regarding the repeat samples being TC and E-coli positive in the distribution system and the well. He was instructed to post a boil water notice. I provided the Tier 1 notice via e-mail and FAQ about boil water notices. I instructed Bruce to submit five confirmation samples from the well. I asked to check with his lab that he had the appropriate sample bottles. I asked him to forward a copy of the notice when it was completed and posted. I spoke to Shawn Stevenson at the Region 2 Springfield office and Brad Daniels in the Portland office. A Level 2 investigation has been scheduled for Tuesday November 27.

Update 01/08/2019:  On 1/3/19 Casey Lyon and I went to the church to meet with Bruce Kuper and other church members. We reviewed both wells and discussed options for confirmed e-coli. We agreed to allow Bruce to begin submitting assessment samples given mitigating factors were found in the well house. We discussed that lifting the boil water notice would require at least three clean assessment samples a few weeks apart. We also discussed that if any sample returned positive for any bacteria we must move forward with a new source or reconstruction. The first sample was submitted on 1/4/18 it returned TC+ on 1/7/18. The church will begin the process of getting the properly constructed well across the street on their property reviewed and approved.

Lifted 09/24/2019:  THE SYSTEM HAS INSTALLED CHLORINE. A DISTRIBUTION SAMPLE WAS SUBMITTED ON 9/17/19 AND IS ABSENT FOR COLIFORM. I CONSULTED WITH SHAWN STEVESON. WE ARE REQUESTING THE SYSTEM TEST MONTHLY FOR TWO MONTHS INSURE THE CHLORINE SYSTEM IS WORKING. THE BOIL WATER ADVISORY HAS BEEN LIFTED.

Update 11/13/2018:  Five of five confirmation source samples are e-coli positive. I spoke to Pastor Bruce at the church. The well will be shock chlorinated on 11/14. We discussed that a well evaluation would occur and that a compliance letter would be coming soon. We discussed the Level 2 investigation scheduled for 11/27.
