| Details: | Routine monthly sample had come back positive for E coli on Jan 14. Three repeats and a triggered source sample were collected this day. Source sample came back positive for E coli on Jan 15. Operator was instructed to issue a boil water notice for residents and nursery staff. He agreed to generate a written notice with provided template and call all residents to ensure they understand. 5 source samples were collected from spring on Jan 15 and submitted to lab, we await results. Hillsboro Pump will shock chlorinate system for operator and then resampling will be conducted on Jan 18. Lifted 03/05/2021: This system has installed 4-log chlorine disinfection and successfully went through plan review for installation with OHA. A routine sample free of E coli and total coliform was taken on Feb 24th and facility has adjusted chlorine injection system to meet appropriate residual at first user. OHA final plan review approval was issued March 5th. Facility has been instructed that they have met all requirements to lift boil water advisory. Operator was instructed to inform residents and nursery staff. |
