| Details: | Mr. Crisp contacted me on 10/23/18 and informed me of the positive repeats. He stated he would be disinfecting the system immediately. I informed him a level 1 investigation has been prompted and he must complete and return to me in the 30 day timeframe. I will also be reaching out to him this day to assure the remainder of the repeat samples are submitted to DWS. | ||
