Details: | Survey deficiency follow up 02.02.2024-Miranda emailed me an electronic version of the Enabling Authority. A copy was sent to the Backflow Prevention Program. The system was asked to follow up with the pending survey deficiencies that have not yet been addressed. 06.02.2023-I emailed Michelle requesting an update on the status of the corrective actions to address the survey deficiencies still pending. 05.08.2023-The system is working with a consultant, Michelle LeBlanc, to address the significant deficiencies and monitoring requirements. 02.26.2023-Conrad Coffey emailed me pictures of the wellhead showing that the well casing was repaired. 01.25.2023-The DWS Compliance office has mailed an Administrative Order to Steve Wagner, the business representative, requesting the water system to correct the significant deficiencies identified during the water system survey and coliform investigation on Aparil 25, 2022. Next steps: Steve was asked to Please address ASAP (no later than February 22, 2023), the actions required to achieve compliance. The requirements are listed on page 3 of the AO. If these actions are not addressed within the next 30 days, the next step for DWS will be a $1,500 penalty. An electronic copy of the AO was emailed to Steve. 12.22.2022-I contacted Steve via email, once again, requesting photo documentation to verify the wellhead casing was repaired. Up to now Steve has not provided the information requested. I advised Steve to address this issue in a timely manner, or if needed, to reach out so we can discuss an action plan to address this deficiency. 12.14.2022-I contacted Steve via email reminding him that DWS will need additional photo documentation of the wellhead. The additional pictures should provide evidence that the section of the casing above ground is watertight and there are no pathways of contamination into the well. |
