Details: Corrective action plan for survey deficiencies
-03.16.2023-Jonathan e-mailed an action plan to address survey significant deficiencies and rule violations:

1-Roof and Access Hatch Not Watertight: Due to wet weather conditions and safety, this is scheduled to be completed by July 1, 2023, to allow for proper adhesion of the seal and safer access. Current attempts have not been successful.

2-Potential Cross connection between the air scouring line and the treated water line: The potential cross connection has been removed. Foto documentation provided.

3-No means to adequately determine flow rate on contact chamber effluent line: Resolved. An inline flow meter was installed on the clear well discharge line.

4-Turbidity profile not conducted on individual filters at least quarterly: Turbidity profiles are now being conducted quarterly. Operator provided a turbidity profile from January 2023.

Next steps: correct roof and access hatch deficiency by July 31, 2023.
