Details: Email contact with state regulators regarding reactivation of system. System was on 4-log compliance monitoring starting in 2011 when OHA required assessment monitoring of wells and/or compliance monitoring. System inactivated and removed chlorination prior to reactivation. System also installed unapproved UV system. Once reactivation occurred OHA asked for system to start assessment monitoring starting in 2023 to monitor source. 4-log compliance monitoring has been removed and assessment monitoring has started. System must complete plan review for UV system and continue assessment monitoring for 1 calendar year.
