| Details: | Survey deficiencies follow up. 04.10.2025- Survey site visit conducted on this day. During the survey the operator completed the ASR for 2023 and 2024. Instructions provided for the operator to create a coliform sampling plan and protocol for under-certified operators. Next steps: complete and submit the CCR for 2023 and 2024, create a written coliform sampling plan and a protocol for under-certified operators. Due date extended to September 2, 2025. 03.18.2024- The plan was reviewed with DMCE. All survey deficiencies precented on the action plan received a time extension for corrective actions to be completed by May 2, 2024. If after this date they remain uncorrected, they will be on the list for enforcement at that point. 03.11.2024-Miranda emailed me an action plan to address the survey deficiencies that are still pending. |
