| Details: | I was notified by DMCE on 4/1/24 that the church was placed back on a monthly coliform sampling schedule due to a failure to submit a Level 1 investigation form on time. This stems from positive routine coliform samples in June and December 2023. I sent the information to the operator on 12/29/23 reminding him that the Level 1 investigation form was due by January 14, 2024. I provided the form as an attachment and hyperlinked the compliance tab in SDWIS for his review. |
