| Details: | Phone call with James Boardman regarding failure to maintain 4-log disinfection for 10 days in Sept. We discussed the treatment failure and when it was back on-line and making sure reports are submitted by 10th of the month. I provided him the public notice (PN) template which he filled out and distributed to all community members (door to door). I forwarded a copy of the PN to the State and provided James with the link to OHA website for all public notice templates and let him know to send us a copy for any contamination or operational/maintenance events. | ||
