| Details: | I first spoke with on-site manager on the phone and he requested an e-mail of all the violations and instructions for corrections. I followed the phone call with an e-mail with the violations and also attached the list of survey deficiencies that have not been corrected. I requested to be put in contact with the DRC who works with the management company and whether or not they have small water system operator. I requested new mailing address since the last letter from OHA DWS was returned due to undeliverable. The on-site manager reported that the office moved. | ||
