Details:	08.12.2025. Optimum Environmental Corp is working with the PWS to address the pending survey deficiencies and unmet rule requirements. The operator has requested a time extension to address the pending survey deficiencies and unmet rule requirements. An action plan was submitted and approved on August 12, 2025. Next steps: -Installation of a 20 gpm flow restrictor on the discharge side of the contact chamber heading to the reservoir. Expected to be completed no later than September 1, 2025. 05.20.2025 The operator provided photo documentation of the corrective actions that have been completed: -HACH 21000Q turbidimeter calibrated - 05.15.2025. Photos provided. -Hole in the clearwell cover sealed with JB tank weld - 05.20.2025. Photos provided. Next step: to ensure CT is calculated correctly, the system has opted to throttle the clearwell discharge pump to maintain a flow rate no greater than 20 gpm. This will be achieved by installing a flow restrictor. 05.14.2025 Survey report and cover letter emailed to operator. DMCE notified. 05.01.2025 Survey site visit completed. Significant deficiencies and rule violations noted.
