| Details: | I received the Level 1 Coliform Investigation report on Saturday (10-11-25) for a series of total coliform positive sample results in September. I reviewed the report on Monday (10-13-25) and requested the form be signed and submitted for entry with no further comments. The two causes cited in the report include: Cause #1: Line break of an illegal hook-up connection on Gooseneck Rd due to work on power infrastructure. Cause #2: Well #1 activation without alert notification alerting the operator to the well being activated without chlorination, thereby, diluting the system with low chlorine residual water in pressure zones 3, 4, and 5 (the well is classified as groundwater and is only required to have chlorine as a residual maintenance and not needed for primary disinfection). Corrective actions included: 1. Boil water notice issued 9/10/25. 2. Disconnection and capping of illegal hook-up 9/09/25. 3. Daily Downstream CL2 monitoring in Zones 3, 4, 5 beginning 9/09/25. 4. CL2 spiking of reservoirs 9/11/25. 5. Proper sanitary collection of samples performed during repeat sampling |
