| Details: | I contacted Mr. Singh to check on the status of the completion of the Level 1 Coliform Investigation form due by 04/25/2019. Mr. Singh stated he never received the letter from Oregon Health Authority Drinking Water Services dated 03/13/2019 with the enclosed Level 1 Coliform Investigation form. He verified the address it was sent to is the correct mailing address. I e-mailed him a copy of the letter and the Level 1 Coliform Investigation form. He said he will submit the completed form to ODA by 04/25/2019. | ||
