| Details: | NCPHD reached out to the operator to provide a courtesy warning about the upcoming survey deficiency deadline of January 22, 2026. Update 1/16/26: I called and left a voicemail for Jeremy requesting a call back to discuss the upcoming survey deficiencies. Update 1/20/26: Spoke with operator regarding survey deficiencies, the operator requested an extension to February 16. The operator needs more time to centralize all the required manuals/protocols and to get the proper certification in place. |
