| Details: | March 24, 2026 survey noted no certified operator on file and no ASR for 2024. Operator also owns/operates Riverview Trailer Park and is DRC. Carolyn used these credentials for Deschutes MP as well. SGWO is good through 7/2029 Carl also submitted 2024 ASR on April 3, 2026. Both of these deficiencies have been corrected. |
