| Details: | I informed Ms. Smith that the system has been issued a second monitoring and reporting violation in the last year for not reporting a routine coliform test for the last quarter of 2019 by 1-10-2020 to the Oregon Health Authority Drinking Water Services (OHA DWS). The first violation was for not reporting a routine coliform test in the second quarter of 2019. The system needs to collect a coliform sample this month (and report it by the 10th of next month) and a second coliform sample sometime this quarter (at least one week after the first sample). Failure to do both of these things will result in the system changing to a monthly schedule of coliform monitoring for at least 12 consecutive months and they will need to meet the eligibility requirements to return to quarterly monitoring. I was told that a coliform sample was collected on 12-11-2019 but it was not reported to OHA DWS by 1-10-2020. | ||
