| Details: | SUMMARY: Water Survey deficiency follow up, significant defeciencies corrected. DETAILS: Mrs. Lori Haury contacted me via email to inform me that all of the significant deficiencies noted in the water system survey had been corrected. She stated that a setback waiver application had been sent to the OHA on January 8, 2015, that she had obtained NSF 60 approved bleach for residual maintenance, created a coliform sampling plan, created an operations and maintenance manual, and an emergency response plan.
