Details:	SUMMARY: Survey Deficiency Corrections DETAILS: Contacted operator by phone and email regarding the 1/25/2018 deadline for survey deficiency corrections. Operator responded on 1/18//18 that the deficiences below have been corrected1. Cover over Spring vault is not insect-proof. Provide secure seal between the top of the vault and the underside of the cover. This will keep insects and small animals from entering the spring. Insure cover is tightly bolted in place. DONE3. The reduced pressure backflow prevention device associated with the boiler has not been tested as required. Contact a person certified in cross-connection prevention to test and tag this device. Device should be tested annually. DONE4. The chlorine level in distribution system is not being tested and recorded two times per week. Now keeping records as required. ACTION NEEDED: None.
