Details: After incident report – Loss of pressure on 06.21.21 through 06.26.21.
Leo Rainwater provided a timeline of events for the water disruption that occurred at the water system.

At 6:15 a.m. on 06.21.21 Steve Klein, RRSA President, contacted Leo to let him know there was no water pressure in the distribution system. Upon arriving at the plant, Leo noticed the water plant was offline, the 75,000 gallon reservoir was empty and the booster pump was running. Water was flowing through the Clearwell and out the overflow. Leo stopped the booster pump and started the investigation with Steve Klein. Leo worked with Steve to develop a plan for restoring the water pressure to run the hydraulically operated diaphragm valves. A leak investigation was also conducted.

No leak was identified in the distribution system. Based on our investigation, it is possible, sometime after 4:00 pm on Monday, June 21, 2021 the hydraulically operated diaphragm backwash valve and the hydraulically operated diaphragm filter effluent valve (Valves 1) opened and allowed the water from the 75,000-gallon storage tank to drain out through the backwash line, through the clearwell and out the clearwell overflow. The root cause of this event is equipment age. To prevent this incident from happening again, the operator will revise the SOP for operating the treatment plant to incorporate the isolation of the backwash pipes within the plant from the storage tank by closing a 3-inch PVC ball valve (Red valve 1 and red valve 2).

Additionally, Jacobs Engineering has recommended replacement of the hydraulically operated diaphragm valves, all solenoid valves and control systems that are associated with the operation of the Backwash and Filter Effluent valves.
