| Details: | Today a voicemail was left for the owner as the facility neglected to submit their monthly coliform sample for November. In addition, it was discussed that the facility has yet to submit their annual source water assessment for 2021. It was also mentioned that the facility still needs to submit their monthly coliform sample for December. Operator returned voicemail and explained that he thought he had completed 12 months consecutively sampling for coliform without an alert. 12/16/21 Today a voicemail was left with the operator explaining that the facility only did 11 months consecutively before stopping. It was explained that the facility must still collect monthly coliform samples through February, then have an onsite inspection conducted before switching them back to quarterly coliform. |
