Details:	SUMMARY: Missing source water assessment smaple DETAILS: Review of Source Water Assessment (SWA) sampling status showed missing or non completion of the 12 months required. Telephone conversation with Mr. Jones regarding all incomplete water quality samples. 12th wqmpl3 for SWA stated to have been done. Mr. Jones to provide lab results. ACTION NEEDED: Mr. Jones to mail to me lab slip. I will forward on the systems behalf to the state. 12/7/11 emailed scanned copy to state.
