| Details: | SUMMARY: Comprehensive Performance Evaluation - Exit Interview DETAILS: DWS staff (Evan Hofeld, Fred Kalish and myself) conducted the CPE exit interview for Midlands plant. Performance Limiting Factors are: 1-no forecasted annual budget or long term plan, 2-No asset management plan, 3-insufficient flocculation, 4-questions about plant flow meter accuracy, 5-insufficient filter bed expansion, 6-highly variable plant flow exceeds max. filter loading rate, 7-heavy reliance on streaming current meter prevents understanding of coagulant dose, 8-questions about estimated clearwell water level measurement , 9-lack of remote plant control capability, 10-lack of written standard operating procedures, 11-jar testing not conducted, 12-adequate disinfection may not be occuring due to demand flow and clearwell volume, 13-backwash period longer than necessary, 14-pro-active maintenance program needed. ACTION NEEDED: Midland to provide corrective action plan and schedule to resolve each PLF to DWS by July 16, 2015. Update: DWS received corrective action plan on 7/15/15. Midland indicated PLFs 1, 3, 4, 6, 11, 13, and 14 have been addressed, with balance of PLFs to be addressed by 12/31/15. 12/3/15: PLFs 2, 8 and 10 have been addressed. 11/29/17 update: PLFs #7 and #9 (2/16/18) addressed, PLF # 5 by 4/30/18 and PLF #12 by 7/31/18. | ||
