The deficiencies noted are as follows:

Finished Water Storage
1. Missing flap-valves at Valley View reservoirs overflows/drains.
2. View Acres East hatch was found unlocked.
3. Damaged screens on vents atop of both View Acres reservoirs.

Monitoring Deficiencies
While Oak Lodge contracts out chemical and biological monitoring to North Clackamas County Water Commission, Oak Lodge must still track the sampling since Oak Lodge Water District is ultimately responsible for the results.
4. Missing coliform sampling plan on file.
5. Missing records of regular chlorine residual monitoring. Residuals must be monitored twice per week in the distribution system.

Management & Operations Deficiencies
6. Missing operations and maintenance manual.
7. Missing written protocols for operators.
