  Comments:
The significant deficiencies and rule violations noted are as follows: 1. The City of Paisely needs to complete a Coliform Sampling Plan (see Attachment 4 – Guideance Documents/Fact Sheets, Coliform Sampling Plan Template). 2. The City of Paisley needs to develop an Operations and Maintenance Manual (see Attachment 4 –Creating a Water System Operations Manual). 3. The City of Paisely needs to complete written operating protocols for other operators (see Attachment 4 – Written Protocol for Operators, fact sheet). 4. The City of Paisely of exceeded the MCL for aresnic and is required to install treatment for arsenic. In addition to the deficiencies listed above and in the survey form, the City of Paisley should address the following recommendations: 1. The Drinking Water Program has established criteria for determining whether a system should be considered to have “outstanding performance.” Systems that are designated outstanding performers may have their water system survey frequency reduced from every 3 years to every 5 years. Although your water system did not meet the established criteria, please review the enclosed handout to see what steps you can take in the future towards receiving this designation. 2. If the City of Paisley is not able to meet the conditions for the installation of arsenic treatment, the water system will need to contact the Drinking Water Program and have the Bilateral Compliance Agreement, dated June 9, 2010, revised with new compliance dates. 3. A summary of your monitoring requirements can be found on page 9. Please maintain a copy of this page and refer to it for future monitoring scheduling.
