Hi Jay,
Missing reports may be e-mailed, mailed, or faxed to the contact information for Compliance Report Submission below.
Nitrate sampling is also due by the end of this year. Alexin lab can do this analysis, so you may want to reach out to them early as there may be delays in sample analysis due to winter weather and COVID19 if you wait until December. This is a sample taken at the hose bib just after the treatment building (same place as entry point chlorine residual, temperature, etc.).

After received some of the missing turbidity reports, I sent the following e-mail on 8/5/2020:
Hi Jay,
We received the reports for the cartridge and UV system, but not the forms for the chlorination system. The form is attached and has a heading that looks like the image below. If you have these, can you send them?
